Product Goals

Our healthcare payment system predominantly reimburses for managing chronic diseases with prescriptions and procedures. This disincentivizes addressing root cause interventions that partner with the patient to address lifestyle contributors. Despite this, we know we need the patient to join the effort in preventing and reversing chronic preventable conditions, but HOW? . Most of us know the basics of health, but promoting change is so hard to do in a regular office visit. In addition to limited time, many providers don’t have the tools to share Lifestyle Medicine advice. Some physicians have tried, but found the advice of “start exercising” or “eat healthier” has not worked. The right approach sparks the change that results in better outcomes.

 

HealthyChange Scripts is a tool that assists physician groups in implementing coach based Lifestyle Medicine into their practice with efficiency and confidence.

 

Patients are introduced to a motivational introduction to lifestyle medicine when they arrive in the office. The brief content is intended to pique interest in a lifestyle change area of their choice. Together, the patient and provider select the script that is pre-loaded with easy, actionable, evidence based lifestyle strategies. The prescription guides the patient conversation with 5 questions using a “health coach” approach based on the neuroscience of behavior change. First step ideas for the provider/patient are listed for reference that help prompt the patient to a doable action step before they leave the office. Additional ideas for behavior change are suggested in small steps to inspire their next step forward. 

 

Physician’s offices are overburdened with treating the preventable symptoms as a result of the standard American lifestyle without the resources needed to address the causes.  Very often the result is a caseload of sicker, more complex patients and burned out providers; a frustrating way to practice.

 

By using the easy HealthyChange Scripts, each visit becomes an opportunity for the patient to be engaged and empowered in improving their health, finally addressing the root cause of disease.  Providers can partner with their patients in a way that builds patient accountability and provider satisfaction.  We all know that work days would be much easier and enjoyable if our patients were free from chronic diseases. We believe that using HealthyChange scripts can assist any provider in implementing the lifestyle behavior change approaches needed in medical care, a change that will benefit the patients, the providers, and the healthcare system overall. 


 

Who can use this product?

 

It is designed for physicians and their support staff to be utilized during office visits, but can be utilized by health coaches, nutritionists, mental health providers and specialists wanting to start the conversation of behavior change to improve health outcomes.

Debby Klein. MS, PT, CHWC

Has been a practicing physical therapist in Buffalo for over 30 years. She earned her BS of Physical Therapy at University of Buffalo in 1993 and her MS of Physical Therapy at Daemen College in 2000. She has been a certified Health and Wellness Coach through Wellcoaches since 2020. In addition to physical therapy and Health and Wellness Coaching Debby ran an adaptive sports program for children and adults with disabilities called the Buffalo Outdoor Sports Association and has volunteered with Girls on the Run which both demonstrated the influence lifestyle has on a person's health and quality of life.  Her vast experience in serving the populations with the most chronic conditions and social and economic barriers to health drives her passion to engage the healthcare system in proactive care so that the joy of health is accessible to all equally.

Dianna Pugliese, PT, CHWC

“Dee” graduated from SUNY at Buffalo with a BS in Physical Therapy. Within 3 years, she became a co-owner of an orthopedic private practice and continued seeing patients and running the practice for 38 years.  She got her certification in Health and Well-being Coaching through WellCoaches in 2016.  She is a member of the American College of Lifestyle Medicine which has been instrumental in helping her gain valuable knowledge about this specialty and the hurdles there are to implementation.  The combination of her time spent in the medical system, passion to learn as much as possible regarding maximizing health and her knowledge of coaching principles, drives her to focus on ways to impact the health of many more people than she could in her private practice.
