Set of the following 12 script pads addressing major lifestyle areas with display packaging
Each prescription pad scripts a mini coaching session that guides the conversation between the provider and patient that results in the patient committing to a first step action plan. It provides a brief benchmark in that lifestyle area and includes an array of activities and behaviors curated to move the patient toward that benchmark.

The scripted questions and curated activities are all derived from research backed coaching techniques and ACLM (American College of Lifestyle Medicine) standards. They have been chosen with the criteria of being accessible and affordable by most.

Healthy Eating: Getting Started:
For the patient that has a low nutrition IQ and needs small, incremental changes. It gives a broad collection of first steps that are relatively easy to incorporate.

Healthy Eating: Next Steps:
​For the patient who is trying to eat healthy and is looking for more information or is deliberately choosing to impact their biomarkers (e.g. A1c, BP, lipid panel, kidney function)

Physical Activity:
For the patient at entry level who sits a majority of the day and is unaware of the harmful effects of being sedentary.
​
Exercise: Upping Your Game
For the patient who is somewhat active and is choosing to incorporate optimized levels of aerobic, strength, balance, and flexibility training.

Stress Less:
For the patient experiencing chronic stress and is in need of tools to actively reduce it.
​
Smoking Cessation:
For the patient that is contemplating or ready to try and break their addiction.
​
Sleep Well:
For the patient who is not being restored by sleep and is unaware of the impact on their brain, body, and emotional health.

Approaching Substance Abuse:
​For the patient who is contemplating and possibly ready to address their substance/alcohol misuse. The style of this script is motivational. The purpose is to guide the patient to the first step toward change giving them options for more structured support with the large list of references provided.

Taking Care of Your Mental Health:
A script designed to link physical, mental, gut, and brain health action steps (like diet, mindfulness, and exercise) to stress management and mental health.

Are You Ready ?:
A motivational interview to be used when a patient is ambivalent about change or has low confidence in their ability to change. It is also included in the accompanying workbook.

Losing Weight: Getting Started:
​For the patient that has not seriously tried to lose weight in the past and has little to no knowledge of their caloric intake or the basics of healthy weight reduction. The education portion includes 8 skills that are keys to success at losing extra fat.

Optimizing Your Health:​
This is designed for people that are on top of their health but are self motivated to further improve their health span and longevity but need additional information. It is an information sharing handout for the patient who already has confidence and has been successful with behavior change.

Body System Reboot: Burn more Fat, Have more Energy, Live Better Longer:
For the patient whose blood work has LDL and triglycerides in the high range, with lower HDLs, creeping fasting blood glucose and HbA1c levels, increasing blood pressure over time, or has high blood insulin levels. They may have physical signs of Insulin Resistance which include (belly, visceral fat) and skin conditions: skin tags at neck, dark velvety skin at neck and armpits, acne, psoriasis, eczema. Subjective complaints include a broad spectrum of the following: chronic fatigue, cravings, joint pain, low energy, constant hunger, brain fog, poor mood, inflammation, weight gain, menopausal symptoms, GI symptoms and poor sleep. This also serves as a weight loss solution for those with Insulin Resistance. It is a longer script of 16 pages as it contains many recommendations for change in multiple Lifestyle pillars.

Weight Loss 2.0: Discover What You Didn’t Know and Design a Better Plan:
A thoughtful 32 page Workbook that is designed to uncover a person’s underlying reasons behind their struggle with weight loss. It guides a patient through a series of questions that will provide insight to the common pitfalls that may have led to previous unsuccessful attempts at weight loss or maintaining weight loss. This comprehensive dive into persistent weight loss struggles encompasses virtually all contributors to lack of success and suggests actionable, accessible steps to overcome them. It includes common categories of weight loss difficulty titled Needing Support, Organizational Planning, Plan for Cravings, Nutritional Realities, Portion Realities, Get Enough Sleep, Track your Movement, Food Addiction Approaches, Mindful Eating, and Mindset.
We created an accompanying prescription pad that helps the provider with recommending the book. It summarizes the book contents and ends with listing the anticipated outcomes including re-igniting motivation and hope, improving knowledge and confidence, and adding self-coaching and accountability to their weight loss journey.

Creating Your Healthy Self: A Workbook:
To be offered to each patient along with the script. It is a self guided coaching instrument that helps generate clear motivations and develops their personal wellness vision. It uncovers personal strengths, environmental and personal supports, a relapse plan, and identifies obstacles. It is part of the accountability and follow through aspect of the plan set forth in the office visit. It increases autonomy, efficacy, compliance and accountability by creating and tracking 3 month and 1-3 week goals in SMART goal fashion. It draws from a coach styled approach in an easy to read, self paced and self reflective style.

Written and Video Patient Invitation:
A document to be read or a motivating video to be shared with a patient prior to the visit (in office or portal). Its goal is to inform the patient of the provider’s move to increasing the Focus on Lifestyle for better health and outcomes and prepare them for the upcoming visit.
It starts by outlining the problem with our “sick care” health system, listing common chronic diseases that are framed as mostly preventable and lifestyle related. Optimal parameters are listed for these behaviors and patients can self-assess their current level of achievement. It prompts them to think about their own health in the future and creates the expectations that they need to play a role in their health and the time is now to begin collaborating with their healthcare team.

(written or electronic) Table formatted page to help patients create and track 1-2 week SMART goals with self rating for success and goal modification as necessary. This template can be used by the provider’s office in portal communications or added to EMR attachments for improved tracking of compliance/success.

Provider User Guide:
For each script there is lifestyle pillar specific information at the physician level to increase provider confidence in the subject. It also includes a description of the purpose of each question on the scripted conversations (for each script) to prepare the provider to respond to the patient for optimal patient engagement with cues to manage time effectively. Included are links to free patient education handouts that can augment the discussion.
Patient Accountability and Referral Guide
These are recommendations for office follow up to strengthen the patient relationship and improve accountability.
For patients requesting or needing additional assistance, an index defining the roles of allied health professionals to refer patients to is included.
